Health Access and SDOH: Mobile Outreach for the Unsheltered Homeless

The COVID-19 pandemic exposed and exacerbated the long-running housing crisis in the United States. The rise in unsheltered homeless encampments in many communities require new partnerships and expanded services in non-traditional settings to reach vulnerable populations in need of both primary care, chronic disease management and connection to housing resources. This national webinar and resource document will outline best practice for mobile outreach services, funding sources and partnership with the local homeless continuum of care.
