Enhancing Care Quality through Community Needs Assessment

Join us for an engaging webinar exploring how Federally Qualified Health Centers (FQHCs) can maximize the impact of their Community Needs Assessment (CNA). FQHCs are required by HRSA to complete a CNA which defines their geography, its population, and the health needs therein. This webinar discusses how to go beyond meeting reporting requirements with the CNA in order to leverage public and FQHC-specific data to improve outcomes for patient populations. Methods discussed include mapping and GIS, as well as innovative utilization of uncommon data sources.

Maryland Supportive Housing Forum 2026

The 2026 Maryland Supportive Housing Forum will bring together more than 300 leaders, practitioners, and partners from across the state for two days of collaboration, learning, and innovation in Baltimore. This statewide gathering will convene policymakers, service providers, housing developers, healthcare organizations, advocates, and people with lived experience to advance Maryland’s vision for safe, stable, supportive housing for all residents. 

Through keynote presentations, interactive workshops, cross‑sector panels, and networking sessions, attendees will explore emerging strategies, evidence‑based models, financing solutions, and policy priorities that strengthen supportive housing systems. The Forum will serve as a catalyst for building shared goals, deepening partnerships, and accelerating Maryland’s efforts to expand housing stability, improve health outcomes, and reduce homelessness. 

By uniting diverse voices and driving forward-thinking solutions, the 2026 Maryland Supportive Housing Forum will help shape the next era of supportive housing across the state. 

This Forum is led by CSH – Corporation for Supportive Housing, with support from The Harry and Jeanette Weinberg Foundation.

Housing as a Foundational Element of Family Well-Being

In 2025, Casey Family Programs, CSH, and Chapin Hall collaborated to convene the Housing as a Foundational Element of Family Well-Being Cohort. This 6-month national learning collaborative was developed in response to growing evidence that housing instability and family homelessness are key drivers of child welfare system contact. Participants included cross-sector leadership from New Jersey, Colorado, Kentucky, and Oregon, who reported that the cohort helped them think more creatively about housing solutions and funding opportunities to better support child-welfare-involved families.

CSH’s Free Healthy Aging Series for Supportive Housing Providers in New York State

In recognition of Older Americans Month in May, CSH is offering a Healthy Aging training series to help build the capacity of housing and human services providers across New York. The series aims to equip providers with evidence-backed interventions and services tailored to meet the unique needs of aging tenants in supportive housing. The program will consist of four 2-hour webinars. Four training modules will be led by topic-specific content experts, and sessions will also include facilitated discussions and opportunities for peer learning. There is no charge to attend, but you do need to pre-register.

  • Module 1 (Tuesday, May 5, 2026 | 2:00 PM – 4:00 PM) will lay the foundations of aging and ageism within the context of supportive housing service provision. Participants will gain an understanding of the holistic health needs of older adults, tools to identify elder abuse, and methods for increasing accessibility. 
  • Module 2 (Tuesday, May 12, 2026 | 2:00 PM – 4:00 PM) will outline the role of case management in addressing tenant health needs, including hospital admission and discharge, medical management, and healthcare coordination. Self-determination, shared decision-making, and legal protections will guide the ways supportive housing providers coordinate, communicate, assess wellness and lead transitions between different levels of care.  
  • Module 3 (Tuesday, May 19, 2026 | 2:00 PM – 4:00 PM) will define proven health practices to prevent overdose and support recovery of substance abuse disorder for older adults. Public health experts will provide an overview of the substance use spectrum, provide non-coercive and dignity-centered case management strategies, and share de-escalation and crisis intervention strategies for service providers.  
  • Module 4 (Tuesday, May 26, 2026 | 2:00 PM – 4:00 PM) will share expertise from a death doula and aging expert on trauma-informed grief support for tenants, staff, and community members. Participants will learn about end-of-life care planning, powers of attorney, advanced directives, and strategies for speaking about and addressing grief and loss.

Webinar – Three Strategies to Protect Medicaid Healthcare Coverage: SOAR, Application Assisters and Medically Frail

Join CSH health and Medicaid experts for another session on changes to Medicaid under H.R.1. These requirements threaten continuous coverage for people who rely on Medicaid for their healthcare and supportive services, especially people with disabilities.

This session focuses on protecting healthcare coverage by leveraging the SI/SSDI Outreach, Access, and Recovery (SOAR) program. We also offer strategies around the medically frail designation and using application assisters.

You will learn:

  • How to use SOAR to help people keep their coverage. Eligible SOAR participants will not need to comply with many of the new H.R.1 requirements (more frequently eligibility checks & work requirements).
  • How states can set up disability categories to exempt medically frail people from the new work requirements.  
  • How states can leverage Medicaid application assisters to keep people enrolled. Application assisters can receive mail and notices on behalf of their clients.
