No matter where we live or what we look like, we all want to provide for our families, have a place to live, and chase our dreams. By joining together, we can help more than a million people and families who are disserved by our fractured public systems, from child welfare to aging. The Supportive Services Transformation Fund (SSTF) can ensure that everyone has the support they need to thrive and a place to call home.

The Transformation fund will create national infrastructure for tenancy supports that can shift us from overreliance on institutional and emergency systems to address housing instability and support people with complex care needs. Specifically:

  • Create a national infrastructure for tenancy supports through Home and Community Based Services
  • Pay for critical services not covered through Medicaid
  • Improve coordination and timeliness of services delivery
  • Build capacity and accountability to address systemic racism

Hear from people who have experienced the impact that services aligned with affordable and accessible housing can have in their lives.

Contact Your Congress Members!

Contact your congress members today to ask them to support the Supportive Services Transformation Fund (SSTF)

Sample text to include in a letter to your Congress Members: As a constituent who wants more home-based care services in my community, I am requesting your support in introducing legislation to create the Supportive Services Transformation Fund (SSTF). The SSTF will make it easier for providers of Medicaid Home and Community Based Services to address housing instability for older adults, people with disabilities, and people with complex care needs. The SSTF will pay for supportive services not billable to Medicaid. States that align the SSTF to housing support services would be fully reimbursed by Medicaid. Please work on behalf of the constituents in your community to establish and fund the SSTF which could eliminate homelessness for people with complex care needs. For more information, please visit Thank you.

Contact the

CSH Policy Team for more information about the SSTF
