About Returning Home
The Returning Home model connects people exiting a state prison system with long-term housing and voluntary support services. The model is highly effective in Ohio at dramatically reducing recidivism and helping people transition back into their communities. Returning Home Ohio is a collaboration between CSH and the Ohio Department of Rehabilitation and Correction (ODRC). ODRC supports the coordination required for persons transitioning out of incarceration including covering costs associated with administering the program.
Launched in 2007, Returning Home Ohio has helped more than 1,100 individuals avoid reoffending, demonstrating the power of stable housing and services.
About the Community Transition Program
The Community Transition Program provides intensive case management services and long-term housing for people exiting a state’s prison system and who are recovering from mental illness and substance use disorder. Similar to the Returning Home Ohio program, Community Transition is supported by and in collaboration with the Ohio Department of Mental Health and Addiction Services (OMHAS).
IN partnership with local organizations, Community Transition has successfully housed more than 1,200 people and ending their cycle of incarceration, mental instability, and substance use.
