CSH is a National Training and Technical Assistance Partner (NTTAP) funded by the Health Resources Services Administration (HRSA) through the Bureau of Primary Healthcare (BPHC). We provide technical assistance to federally qualified health centers and other organizations working in tandem with health and housing, particularly those that serve individuals facing housing instability, homelessness, or residing in public housing. As part of our training and technical assistance work, we also produce several national resources like learning collaborative, national webinars, publications, tools, resources, and direct technical assistance. Key focus areas include:

  • Health and Housing Partnerships
  • Social Determinants of Health
  • Housing and Urban Development (HUD) Homeless and Housing Programs
  • Supportive Housing Development

Frequent Users Systems Engagement (FUSE) Learning Collaborative:  Learn an Innovative Approach to Data Matching

CSH’s signature Frequent Users Systems Engagement (FUSE) approach is a proven model identifying frequent users of hospitals, jails, shelters and/or other crisis public services and then improving their lives through supportive housing. For health centers that are screening and documenting for SDOH, FUSE provides a pathway to use health and other system data and establish effective housing partnerships to improve health and housing outcomes for patients that experience homelessness and have other co-occurring health and behavioral health conditions.

The Key Ingredient for Diabetes Prevention and Control Programs

Community Health Workers and Peer Specialists are trusted members of the community who empower their peers through education and connections to health and community resources in diabetes prevention and control. CSH and MHP Salud, HRSA training and technical assistance providers produced this fact sheet Peers and Community Health Workers (CHWs): Key Ingredient for Diabetes Prevention and Control Programs to highlight a multi-year examination of key benefits CHWs bring: 1) lived experience able to customize health information and instructions 2) bridge to the broader diabetes care team and 3) advocacy for the needs of their clients. This fact sheet presentation will benefit health center leadership, clinical staff and front-line workers to advance the role of CHWs in diabetes prevention and control.
