RFP for Training and Technical Assistance to Health Centers & Their Partners

CSH is seeking an organization to provide training/technical assistance (T/TA) to health centers and health center partners through a United States Department of Health and Human Services (HHS) funded initiative. This HHS funded TA initiative focuses on diabetes control and diabetes management for homeless and vulnerable populations, screening and addressing social determinants of health (SDOH), and community partnerships for health equity.

CSH is seeking assistance primarily in the area of diabetes management and control. The appropriate organization(s) will need to demonstrate that they have the following qualifications:

  • Experience working with health care centers that service homeless populations;
  • Content matter expertise on health and healthcare issues for homeless and vulnerable populations, specifically diabetes;
  • Can access and utilize national healthcare data;
  • Expertise in providing training and technical assistance to health care centers on health and housing;
  • Experience working with departments of the federal government.

The organization will be expected to complete the following tasks:

  • Outreach

The subcontractor will provide CSH with recommendations for health centers to engage with and target for initiative work.

Deliverable: Connect 50 Health Centers to initiative work.

  • Webinars and Learning Collaboratives

Coordinate, collaborate and lead with CSH webinars and learning collaboratives under the initiative workplan.

Deliverable:  Collaborate with CSH and/or lead on at least two webinars and two learning collaboratives, specifically around diabetes management and control for homeless and vulnerable populations.

  • Content Development
    Development of original materials and review of CSH-authored materials related to initiative workplan deliverables; needs assessment utilizing UDS data; assist with the development of tools, guides, and best practices models.

Deliverable: Provide data analysis and content on at least two written guidance documents.

  • Project Management
    Participate in regular team calls with CSH; provide other collaboration as needed; and complete required project reporting.

Deliverable: Monthly initiative call participation and reporting.


Funding Available:

An amount not to exceed $100,000 is available for this project.

Contract Time-frame:

The contract will cover one-year of the HHS initiative (July 1 2020-, June 30, 2021).

Application Process:

This information includes proposed billing rates, completed copies of the forms referenced above, resumes for up to six proposed staff members included in the Organizational Experience matrix, anda two paragraph description of your organization's core competencies and accomplishments.

Evaluation:

CSH will evaluate applications utilizing the following criteria and will select the valid proposal:

  • Demonstrated successful performance of substantially similar work:
  • Relevant project work:
  • Experience and expertise;
  • Years of relevant experience: and
  • Rate reasonableness and overall cost of services

Organizations must be able to comply with the administrative and financial management requirements of the prime award from The Health Resources and Services Administration including the financial management standards for funds control and accountability and the U.S. Department of Health and Humans Services Grants Policy. Organizations must have an active registration on the System for Award Management found at sam.gov and cannot have existing, pending or expired debarments that preclude them from doing business with the United States government and cannot have convictions for, nor have any pending indictments for, fraud or a criminal offense in connection with a public contract or subcontract.
