Chicago Supportive Housing Provider Medicaid Capacity Building

Through the generous support of the Polk Bros. Foundation, CSH has engaged with the National Council on Behavioral Health and MTM Services, Inc., to help Supportive Housing providers in determining their potential for revenue from Illinois’ Medicaid expansion made possible through the Affordable Care Act.

CSH seeks to engage approximately 10 organizations to complete a cost, revenue and
capacity analysis related to their supportive housing services for Medicaid-eligible
populations. The approximate timeframe of the initiative is November 2014 through June
2015.

Interested organizations should complete the attached application and send to Betsy Benito. Technical assistance will be provided at no-cost to Chicago-based providers, with preference given to Polk Bros. Foundation grantees. Organizations outside of the City who are interested should reach out to Betsy Benito for further discussion.

This initiative will begin on November 20, 2014 with a webcast that is from 10 a.m. to 1 p.m.
