About the subgrant: Feasibility assessment for a Pay for Success structure to create supportive housing focused on super utilizers of health or other crisis resources.

CSH and its partners will work with the Volunteers of America Delaware Valley to assess the feasibility of a Pay For Success initiative focused on creating supportive housing for vulnerable populations. Specifically, this project anticipates targeting super-utilizers with two or more (non-surgical, non-OB) hospital admissions within six months and two or more chronic illnesses who are also homeless or experiencing housing instability.

Among other technical assistance, CSH anticipates working with the Volunteers of America Delaware Valley and its partners including the Camden Coalition of Healthcare Providers (CCHP) to integrate individual level health care claim data with homelessness data, allowing for analysis and segmentation of the overall population to identify a subset of hospital super-utilizers who are homeless. In addition, CSH will work with the Volunteers of America Delaware Valley to increase the capacity of local supportive housing providers to work with the vulnerable populations and to follow a high fidelity housing first model. The geographic focus is the City and County of Camden, New Jersey. The Camden Coalition is poised to become a Medicaid Accountable Care Organization (ACO), enabling it to receive shared savings through ACO contracts with Medicaid managed care organizations (MCOs) based on improved quality and reduced costs.

CSH’s Role: In addition to providing technical assistance, CSH is providing Volunteers of America Delaware Valley with a $15,000 subgrant.

We are grateful to CSH and CNCS for allowing us to continue our valuable partnership with the Camden Coalition of Healthcare Providers and work towards the implementation of the Pay for Success initiative. Housing instability is a very real problem in Camden and we welcome the assistance in identifying those most vulnerable so that together we can move towards providing that population with housing as well as supportive services.” - Daniel L. Lombardo, President/CEO of Volunteers of America Delaware Valley
